  • Activity: Normal around house or school
  • Medical: Readily available

Class 2 Medical Form

3 years

• Doctor fills-in and signs.
• Required for all activities.
  • Duration: Over 72 hours
  • Activity: Normal around house or school
  • Medical: Readily available
• For people under the age of 40.
• If you are 40 or older you must use the Class 3 instead.

Class 3 Medical Form

1 year

• Doctor fills-in and signs.
• Required for all activities.
  • Duration: Over 72 hours
  • Activity: Above normal
  • Medical: NOT readily available
• Required for BSA Physical Fitness Award.
• Required for Personal Fitness Merit Badge.
  • Also requires a note from your Dentist

BSA Physical Fitness Award

1 year

• Doctor signs granting approval to start fitness program.

• Some BSA camps like Florida Sea Base, Northern Tier and Philmont Scout Ranch have their own medical forms that must be used.
